統合ケアの概念モデル『カオスの虹』について - 岡山の家庭医の読書・勉強ブログ

今更になってようやく、統合ケアの「カオスの虹」がすこしわかってきた気がする。

支援機能の連携である機能的統合（Functional integration）と、文化や価値観の共有をおこなう規範的統合（Normative integration）を基盤としながら、ミクロレベル（個人を対象）では臨床的統合、メゾレベル（グループを対象）では専門職統合や組織統合、マクロレベル（住民レベルを対象）ではシステム統合をおこなっていく。

方向性として機能的統合の側面が強ければ、Triple Aim OutcomeのうちのCost ＆ Utilizationが達成されていき、規範的統合の側面が強ければExperience of Careが達成されていく。Triple Aim OutcomeのうちのPopulation healthは機能的統合と規範的統合のどちらも必要となる。
